The University of Oklahoma, Department of Anesthesiology provides verification for completion of Internship, Residency and Fellowship Training in addition to Professional References for previous University of Oklahoma, Anesthesiology Residents and Fellows. There is an administrative processing fee required for verifications and professional reference requests. Verification services are rendered only after the processing fee payment is confirmed.  Please allow our office at least 5 business days to complete and return your verification request once payment is confirmed. 

The following list, identifies the processing fee for each type of verification service. 

**Training verifications for current trainees or graduates within 60 days of their graduation date are provided at no-cost.**

Verification of Training and Professional Reference Fees:

• $75.00 - Internship & Residency Verification 
• $75.00 - Fellowship Verification 
• $50.00 - Professional Reference Verification

• Institution Name requesting Verification 
• Contact Name completing request 
• Contact number for the Verification requester 
• Email for the Verification requester
• Resident Full Name 
• Degree MD/DO 
• Dates of training

Once the administrative processing payment is confirmed, Verification requests will be returned to the requestor within 5 business days  ( Verifications for Graduate Training completed prior to the year 2004, may take up to 10 days to complete)
