OUHSC Office of Admissions and Records

The permanent academic record accumulated by students in the OU College of Medicine is maintained by the Registrar in the OUHSC Office of Admissions & Records. Upon written request, copies of the student’s transcript will be made available to the student. The Office of Admissions and Records is part of the Office of the Vice Provost for Academic Affairs and Faculty Development. They provide support services and information for prospective and current students, alumni, and University administration.

Information maintained by the University of Oklahoma about students and, in some instances, former students are covered under the Family Educational Rights and Privacy Act (FERPA).  FERPA is defined as either directory or confidential. Any office gathering such information and/or having custody of it shall release it only in accordance with this policy or as otherwise required by law. Read the complete FERPA policy.

Release of Student Information

Information maintained by the University of Oklahoma about students and, in some instances, former students are covered under the Family Educational Rights and Privacy Act (FERPA).  FERPA is defined as either directory or confidential. Any office gathering such information and/or having custody of it shall release it only in accordance with this policy or as otherwise required by law. Read the complete FERPA policy.

Directory Information

This is information, which routinely appears in student directories and alumni publications and may be freely released.  Upon written request by the student, directory information will be treated as confidential and released only with the student's consent except where disclosure is required or permitted by law. 

  • Students may at any time request that directory information be treated as confidential and released only with their consent.
  • Students should contact the Registrar at their specific campus location for the appropriate forms.
  • Students should be advised that by withholding directory information, University officials are prohibited from releasing any form of information without their consent, or as permitted or required by law.  This means the status of students, who apply for an auto loan, good student discount, apartment lease, employment verification, enrollment verification, loan deferment, etc., will only be verified by University officials when accompanied by a release from the student. 

Types of Directory Information

The University of Oklahoma, in compliance with the Family Educational Rights and Privacy Act (FERPA), has designated specific information as Directory Information:

  • Name
  • Home & Permanent Address
  • Email Address
  • Telephone numbers
  • Major field of study
  • Class year
  • Enrollment status
  • Anticipated degree date
  • Participation in officially recognized University activities
  • Degree and awards received (including outstanding or recognized academic achievement)
  • Most recent previous educational institution attended

Although not specifically included within the definition of directory information, the University permits faculty to post individual student grades and interim class evaluations provided the information is identified by code numbers and does not identify the student.

Students who do not want this information released or posted must notify their instructor or the Registrar's office.

Non-Directory Information

The following is considered non-directory information and cannot be released without consent.

  • Student ID #
  • Date and place of birth
  • Student maiden name
  • Photo

Confidential Information

This is all other information contained in the student's educational record that can be released only upon the written consent of the student, with the following exceptions defined in the Family Educational Rights and Privacy Act of 1974, as amended, which waive prior student consent.

Transcript Requests

Electronic official transcripts are obtained through the National Student Clearinghouse ordering site. Hardcopy versions of the official transcript are also available. Learn how to request an official transcript.

An unofficial transcript is obtained through the Student Self-Service at ouhsc.edu/selfserve

Enrollment Verification

Current students may print enrollment verification through Student Self-Service at ouhsc.edu/selfserve or by contacting the Office of Admissions and Records.

Letters of Good Standing may also be requested from the Office of Admissions and Records (admissions@ouhsc.edu). Always include your student ID number when contacting the Office of Admissions and Records.

Graduation and Diplomas

Graduation

  • Students must meet certain criteria with their College and the University. Read about University Graduation Requirements here
  • College of Medicine Graduation requirements can be found on the Academic and Education tab of the Student Handbook.
  • Students are required to Apply for Graduation by March 1 for Spring Graduation. December Graduates should apply by October 1. Access to the Graduation Application is available through ouhsc.edu/selfserve.

Diplomas

  • Diplomas are available to students approximately 6-8 weeks after their graduation date.
  • All Bursar accounts and Financial Aid holds must be cleared before the Office of Admissions and Records can release the diploma.  Student requesting a replacement or duplicate diploma may contact the Office of Admissions and Records at (405) 271-2359 or by email at admissions@ouhsc.edu to request the Affidavit for a Replacement or Duplicate Diploma.
  • If your name has changed and you would like your diploma reprinted with your new name, you must submit a copy of the court order documenting your legal name change, the Affidavit for Replacement or Duplicate Diploma, and the $50.00 fee to the Office of Admissions and Records. 
  • Digital Diplomas can also be requested through Parchment for students who graduated during the 2021-2022 year and beyond.
