The Department of Orthopedic Surgery and Rehabilitation provides verification of residency and fellowship training, and professional references for current faculty and former trainees. Verification services are rendered only after payment has been confirmed. Please allow our office at least 7-10 business days* to complete and return your verification request once payment is confirmed. 

Verification Fees**

• $100.00 - Residency Verification
• $100.00 - Fellowship Verification
• $75.00 - Professional/Peer Reference Verification

Information needed for verification requests:

• Name of institution requesting verification
• Name, phone number, and email address of the person requesting verification
• Full name of person being verified 
• Degree (MD/DO)
• Dates of training

Once the verification fee has been processed and payment is confirmed, your verification request will be returned to the requestor within 5-7 business days.

*Verification for graduates who completed their training before 2004 may take up to 12 business days to complete
