Limited Competition: Institutional Development Award (IDeA) Program Infrastructure for Clinical and Translational Research
We propose to strengthen productive bidirectional relationships between researchers and communities in Oklahoma with an emphasis on primary care clinicians, pharmacists, and American Indian communities, expand community test sites by enhancing three practice-based research networks (family practice/general internal medicine, pediatrics, and pharmacy), and build a system wide dissemination/implementation/diffusion infrastructure consisting of county health improvement organizations (CHIOs) linked to each other and, through regional coordinating centers, to the academic research partners. We will also provide training and technical assistance to researchers and clinicians affiliated with the University of Arkansas for Medical Sciences and to the Oklahoma City Area Inter-Tribal Health Board, which includes 43 federally recognized tribes and at least 38 tribal and IHS health systems and facilities located throughout Oklahoma.

Leveraging Practice-Based Research Networks to Accelerate Efforts Implementation and Diffusion of Chronic Kidney Disease Guidelines in Primary Care Practices.
We propose to facilitate, using evidence-based strategies, implementation of chronic kidney disease guidelines in one practice in each of 32 different communities in four different states. We will then help each of those practices spread the guidelines to two additional practices within those same communities, reaching a total of 96 practices. Strategies will include performance feedback, academic detailing, practice facilitation, IT support, local learning collaboratives, and inter-practice site visits.

The goal of this project is to improve the management of obese patients in primary care by 1) supporting linkages between primary care practices and community-based resources to support obesity treatment and management; and 2) creating an implementation guide on obesity management to be disseminated widely to diverse primary care settings that describes optimal processes for developing, assessing, and maintaining effective linkages with community-based programs.

The goal of this project is to connect primary care practices, that are already using electronic health records and are linked through a local health information exchange hub, to a regional health information organization called SMRTNET, which will provide access to a broader range of information, from a variety of sources, including hospital records, laboratory tests, pharmacy records, and a statewide immunization registry. It is anticipated that access to this additional health care information will reduce unnecessary laboratory testing, improve medication reconciliation, reduce hospital readmissions, decrease some practice administrative costs, and improve patient satisfaction with continuity of care.

Creating a New Model for Delivery of Preventive Services
The major goal of this project is to establish then study, within a single primary care practice in Weatherford, Oklahoma, a model, evidence-based preventive services delivery system. The model system, which will substantially separate preventive services from routine office visits, will include brief, periodic “longevity visits” between physician and patient followed by longitudinal interactions between the patient and a preventive services nurse. The entire process will be facilitated by a computerized patient registry with decision support, recall and reminder, and audit functions.
