The Petition for Prerequisite Substitution form is to be used by applicants who wish to petition for substituting college-level credit from U.S. institutions for prerequisites of the University of Oklahoma Physician Associate Program. This form does not need to be completed unless you are petitioning a substitution. Please submit supporting documentation with our petition. You can submit either a course description or a course syllabus.
